Where to Report A Clery Crime

The Clery Incident Reporting Form should be used by all CSAs for non-emergencies. The form works for all UAA Community Campuses.

Incidents of gender-based and sexual misconduct should also be reported to the Title IX Coordinator.

This form is NOT intended for emergency situations such as an immediate safety concern. If you are reporting an emergency situation, please call 911 and then notify a campus official.

Reports are directed to the following campus officials:

For the Anchorage Campus and Aviation Technology Complex reports are directed to the University Police Department.

For Kenai Peninsula College - Kachemak Bay Campus, reports are directed to the Campus Director or the Student and Enrollment Services Manager

For Kenai Peninsula College - Kenai River Campus, reports are directed to the College Director or the Student Services Director

For Kodiak College, reports are directed to the College Director or the Student Services Representative

For Matanuska-Susitna College, reports are directed to the College Director, the Director of Student Services, the Director of Academic Affairs, or the Theater Director

For Prince William Sound College - Valdez Campus, reports are directed to the College Director or the Director of Student Affairs

For Prince William Sound College - Extension Centers, reports are directed to the Extension Center Director or the College Director
